Dear Colleagues

I wondered whether anyone had put together a basic functional skills checklist (E.G. can do standing transfers, can load a spoon, can crawl, can pincer grip a pencil etc) to aid transition/ or even be used as an observational assessment for us. Although I've done my utmost to encourage PD specific care plans, they are very often not completed, meaning the quality of transition TACs are dependent upon remembering to ask all the right questions and getting all the right people to attend. Given that my school SENDCos need to prove every penny and EHCPs are generally becoming harder to obtain, it's becoming obvious to me that something like this could prove an important starting point for receiving schools and possibly for me to occasionally measure 'progress'/ identify possible next steps..

 

Any thoughts?

While school staff can use such systems to identify progress and flag potential issues, I would suggest that as soon as issues with physical development are identified as a concern then referral should be made for OT assessment and that the OT can then assess the pupils abilities and needs as well as make suggestions for adaptations required to support the pupil.

Active Hands are Achieving Hands is a useful booklet OT Janet Taylor in the South Derbyshire team put together. I'm uncertain if this can be purchased but might be worth contacting the Team.

One of my PI teachers has created a resource pack to use along side the booklet and advise from the North Derbyshire team.

This teacher is also thinking about adding a checklist to accompany the resource pack.

I will keep you up to date as I find out more. Im not technically minded enough to be able to post photos I'm afraid.
