Sorry, not a plan but I downloaded a document from the resources section called 'Inclusion in Physical Education and School Sports' that I share a lot with schools. It has great ideas for adaptions to including wheelchair and frame users (which in turn is helful for lots of pupils with PD) in most units of PE and a section on sports day. Worth having a look.
