You need to do the survey at the end of the modules before it generates your certificate.

I did, then did the course again and tried to do the survey again but it states I have already done the survey.

I did the survey after I finish the course and I did not get my certificate

I also completed the self assessment and then clicked through to the certificate, but it was a blank screen.
